The Clinical Value of Laparoscopic Appendectomy for Complicated Appendicitis, Such as Perforated Periappendiceal Abscess and Overweight Patients

Abstract
|
|
|
Purpose: Many authors have reported on the effectiveness of laparoscopic appendectomy (LA). However, there is still an argument for the clinical value of this procedure for treating complicated appendicitis and overweight patients.
Methods: One hundred forty patients who received laparoscopic appendectomy at Sanggye Paik Hospital between June 2004 and July 2006 were included in this study. For comparison, 140 open appendectomy (OA) cases were randomly selected during the same period. Clinical data such as age, gender, operation time, the first oral intake, the hospital stay and the complication rate was retrospectively reviewed for the complicated appendicitis group and the overweight patient group.
Results: For the overall 280 cases, 71 cases were classified into complicated group. The OA group had 33 cases and the LA group had 38 cases. The mean operation time was not different between the OA and LA groups (p＞0.05). The mean first oral intake time and hospital stay were significantly shorter (p＜0.001) in the LA group with complications. Seventy seven cases were identified as being overweight via the body mass index (BMI)＞23 kg/m2; the OA group had 42 cases and the LA group had 35 cases. The mean operation time was longer in the LA group (p＜0.05). Yet the mean first oral intake time and mean hospital stay was significantly shorter (p＜0.001) in the LA overweight patient group.
Conclusion: Laparoscopic appendectomy is a very effective and favorable procedure for treating complicated appendicitis and overweight patients, as well as for treating simple appendicitis.
